If you leave your managed care plan, you will need to choose a primary care provider. You will still belong to a regional organization that can help you with physical and behavioral benefits.

To opt out of your managed care plan and to choose a primary care provider, visit enroll.healthfirstcolorado.com or call Health First Colorado Enrollment at 303-839-2120 (in Denver), 1-888-367-6557 (outside of Denver), or State Relay: 711 for members with hearing or speech disabilities, Monday-Friday, 8:00 a.m. to 5:00 p.m. (closed state holidays).

The Health First Colorado Enrollment number is not for information on benefits or to find out if you qualify.

Chatbot. You can use the chatbot on CO.gov/PEAK or Health First Colorado to find your renewal date. Start the chat and choose “Check my benefits.” After confirming your identity, you can access your renewal date.

For Current Members Who Need a Replacement Card

Option 3: You can also call or go in person to your local County Department of Human Services to request a card be mailed. The card will take up to two weeks to arrive.

Option 4: Finally, you can call the Member Contact Center to request a card be mailed. When calling, choose option 5 to speak to a representative about ordering a replacement Health First Colorado card. Once you’ve completed the process the card will be mailed to the address on file and should arrive within two weeks.

  1. It is a medical emergency.
  2. Your health would be endangered if you were required to return to Colorado for the medical care/treatment.
  3. The health care provider that treats you must enroll in the Health First Colorado program. Health care providers can visit the For Our Providers page for more information.

If you have more questions about seeing a health care provider out of state, call the Member Contact Center.

  1. Contact your local county office or Medical Assistance site.
  2. If you are enrolled in Health First Colorado (Colorado’s Medicaid program) or Child Health Plan Plus (CHP+) and do not receive food assistance, cash assistance, or financial assistance to purchase private health insurance through Connect for Health Colorado, you can also call the Member Contact Center at 1-800-221-3943 / State Relay: 711 to change your name.

After you make the updates to the packet, sign the Renewal Form Signature Page, and return the form to your local county human services department by your renewal deadline.

If you do not have changes to your information, sign and return the Renewal Form Signature page to your local county human services department by your renewal deadline.

Important: If you do not return the Renewal Form Signature Page by the deadline, you may lose your health care coverage.
